Use Edit to build a custom intake form for the patient to fill out when they book online.   Add a new question or use edit to modify a default question.   Enter your question text, set the control (answer) type, choose if the answer is required, and check the box for online scheduling. Marking active will show the field in the communication center. Marking Online Scheduling will show it on the booking page.
