How To Add Custom Fields to the Lead Profile
Objective
To provide a clear and concise process for team members to set up, manage, and customize the lead profile questionnaire in the client management system, ensuring accurate data collection and improved patient onboarding.
Key Steps
- Access the Client Manager
- Log into the client management system.
- Navigate to the “Patient Scheduling Onboarding” section.
- Locate the Questionnaire Tab
- Click on the first tab labeled “Questionnaire.”
- Edit the Questionnaire
- Click on the “Edit” tab next to the questionnaire to load existing custom fields and questions.
- Review Default Questions
- Familiarize yourself with the default questions provided, which typically focus on insurance information.
- Add Custom Questions
- To add a new question or field, select the option to create a new entry.
- Choose the appropriate control type (e.g., text box, dropdown) for the answer format.
- Mark the field as required if necessary.
- Activate Questions
- Ensure that the new questions are activated to appear on the lead profile and can be included in the patient intake form if applicable.
- Deactivate or Delete Questions
- To remove a question from display, deactivate it. Note that questions previously answered cannot be deleted but can be deactivated.
- If a question has never been answered, you can delete it entirely.
- Reorder Questions
- Adjust the order of the questions by dragging and dropping them in the desired sequence on the left-hand side.
- Modify Existing Fields
- Edit any previously created fields as necessary to ensure they meet current needs.
Cautionary Notes
- Historical Data Preservation: You cannot delete questions that have been answered to maintain historical data integrity. Instead, deactivate them.
- Required Fields: Be cautious when marking fields as required; ensure that they are necessary for your data collection process to avoid frustrating users.
By following these steps, team members will be able to efficiently manage the lead profile questionnaire, leading to improved data accuracy and enhanced patient onboarding experiences.
